Oklahoma is expanding the use of a community referral platform designed to connect healthcare and community organizations with resources addressing needs such as food, medical care, utilities and other support.
Helpful for: Locating community programs and connecting individuals with available services.
Truth is researching: Which practitioners know about and use this system, how referrals are completed, and what information returns to the referring organization.

SoonerCare Health Management Program
The Oklahoma Health Care Authority's Health Management Program provides health coaching and care-management support for eligible SoonerCare members and can help connect members with healthcare providers and community resources.
Resources may include connections involving preventive care, transportation, food security, housing and other needs.

SoonerCare Choice uses a patient-centered medical-home model in which members are connected with a primary-care provider who coordinates care and referrals.

SoonerCare Nutritional Services
SoonerCare provides coverage for certain medically necessary nutritional services delivered by qualified registered dietitians when applicable program requirements are met.
Coverage and eligibility requirements may vary based on the individual's circumstances and medical need. Confirm current eligibility and coverage directly with SoonerCare.

Finding Community Help
Not sure where to start?
Oklahoma has multiple public, nonprofit and healthcare resource systems. Truth in Health is working to make those pathways easier to find and understand.
As this guide grows, resources will be organized by need—including nutrition and food access, aging and caregiver support, transportation, healthcare navigation, rural resources and other community services.
A Resource Existing Is Only the First Step
A program can exist without the people who need it knowing about it. A practitioner can make a referral without knowing whether the patient was able to receive the service. And community organizations may have valuable resources without a reliable way to communicate them to healthcare practitioners.
Truth in Health is interested in the entire connection: resource awareness → referral → access → service → communication → follow-up.
For Oklahoma Practitioners
Do you regularly refer patients or clients to community resources? Truth in Health wants to better understand what happens after that referral.
